Ammonia poisoning is caused by an aquarium that has not been cycled, overstocked fish and not enough water changes.

Prevention :
Avoid adding too many fish to your aquarium at one time, complete your aquarium's nitrogen cycle before adding fish and do a weekly water change of 20%.
